For physicians and care teams
If I've called your office about a patient, here's who I am.
I'm Whitney Treloar, MSN, RN, CCM. A patient's family has hired me privately to help coordinate their care. I work remotely and I'm not part of any practice, hospital or insurer.
My role
Coordination, education and advocacy. I help the patient and family understand and follow your plan: keeping medication lists consistent across prescribers, preparing questions before visits, and making sure instructions are understood afterwards. I don't diagnose, prescribe or direct treatment.
When I call
I'll usually ask for the nurse line. It's a handoff, nurse to nurse. I don't call to chit-chat: if I'm calling, it's something I think the provider should see. What to do about it is the provider's call.
Authorization
I only discuss a patient with your office when I have written authorization from the patient or their legal representative. I'll send a copy to your office before we talk.
Licensure
Registered nurse licensed in Florida (multistate compact), New York, California, Illinois and Nevada. Certified Case Manager (CCMC).
Concierge practices
If your patients' adult children live out of town, I can be the person who keeps them informed between visits, so your team isn't fielding those calls. Email me athello@mynursewhitney.com.
Background
I've been a nurse in hospitals, in patients' homes and on the phone: ICU and cardiac floors, home health, psychiatric case management, and remote case management. Most recently I've managed teams of nurses, therapists and social workers caring for older adults at home.
Honestly, I need things broken down into their simplest form before I really understand them myself. That turns out to be one of my strengths: it's how I explain things to patients and families. I also taught nursing at the college level, and I have a master's in nursing education.
